Who do I list as my vaccination provider?
Because all vaccination clinic sites are administered under the care and CalVax license of Urgent Care Now and Dr. Rusty Oshita, you can add the main clinic as the vaccination provider. This may come up as a question when traveling to Hawaii or other jurisdictions.
